JH/HS Rebel Sports Physicals

For the 2023-2024 school year, all incoming 7^th, 9^th, and 11^th grade athletes will be required to get an updated sports physical.

Physical forms will be due by August 21, 2023.  (High School Volleyball physical forms due July 31st)

Dr. Warren Worsham will be doing sport physicals at SR on Saturday, July 8, 2023, from 9 AM – 1 PM. No appointment is necessary; first come, first served. The cost is $15 per student athlete. 

To print a copy of the SRISD Physical Examination Forms, please click here. The first page of the form must be completed by a physician or one of the other medical professionals listed in the box on the bottom of the first page. The second form needs to be completed by the student’s parent/guardian.
