JH/HS Rebel Sports Physicals

For the 2022-2023 school year, all incoming 7^th, 9^th, and 11^th grade athletes will be required to get an updated sports physical. Physical forms will be due by the first day of the 2022-2023 school year in order for the athlete to be able to participate in practices and games. 

To print a copy of the SRISD Physical Examination Forms, please click here. The first page of the form must be completed by a physician or one of the other medical professionals listed in the box on the bottom of the first page. The second form needs to be completed by the student’s parent/guardian.
